Child care
will be provided Monday through Friday.
Care will begin when school is dismissed and
will close promptly at 5:00. Parents
who are late picking up their child will
result in dismissal of their child from the
Dyer Child Care.

All enrollment
forms for the child MUST be completed by the
parent on admission.

Parents must
state on the admission form if anyone else
is to pick up the child- listing persons who
may pick up their child, and giving
authorization to do so.

Children may
bring their toys, but they are responsible
for them.

Children will
be given an afternoon snack.

Outdoor play
is an important part of the program. We
will be using the playground at school for
play when the weather is suitable.

Parents are to
notify me of change of work or home phone
numbers.

Dyer School
Care will operate on school schedule.
Anytime that school dismisses early because
of snow or school schedule there will be no
child care that afternoon.

Child Care
fees are as follows:
$20 per week $15 for
second child (afternoon only)
$20 per week $15 for
second child (morning & afternoon
$10 per week $5 for
second child (morning only)
$5 per day
$5 registration fee is required
on admission

Fees must
be paid at the beginning of the week.
Payday is on Monday. If their is a snow day
a deduction will be taken from the next
weeks balance. Failure to pay will result
in dismissal.

Every child
must show proof of insurance. Insurance is
available through the school if you wish to
buy it. Forms for the insurance will be
sent home through the school.
